Currently, there is no button on the WAS page to apply for 5 Band WAS. It is currently on the web site update which is expected to be done in 2012. In the meantime, please follow these steps for applying for 5 Band WAS:

Before anything, you must first qualify for 5B WAS with 50 states on each of the 5 primary bands of 80, 40, 20, 15 and 10 meters.

Sign on to the LoTW site then select Awards;
Click on Select WAS Award Account;
On the left click Application to start the application process;
Next, select each of the 5 primary bands as endorsements only;
Click Continue at the bottom of the page to bring you to the next screen;
On Part 2 make sure you check here the Endorsement button;
If this is a hybrid application it is here where you will complete the paper portion by noting the callsigns
     in the box(s) next to each of the State names corresponding to the proper band(s);
Also, note if you are sending cards to ARRL HQ or, an HF Awards Manager;
Click Continue at the botton of the page to take you to the next screen;
On Part Three here is where you complete the personal information LoTW needs for your application.
     Normally, this information needs to be completed on your first-ever application, but, for subsequent
     applications please check this information to be sure it is the most current;
Click Continue at the bottom of the page to take you to the next, and final screen;

Very Important:

On Part Four in the upper portion in the comment box note here something like this:

This application is only for 5 Band WAS;
Note here if you also want to purchase the (optional) wall plaque and lapel pin;
(Also not here anything you want to bring to my attention regarding this application;

The lower portion of the screen is where you note payment:

If you are a member of the ARRL Diamond club or Maxim Society click this box.

Special note: Remember, ONLY check this box for Diamond club if you are Level four (Gold) or higher. If you are Honorary, or levels 1, 2 or 3 please do not check this box. Free awards apply ONLY to level 4 or higher;

If you select payment by mail or fax this MUST be received at the WAS desk within 30 days. If payment is not received within 30 days your application may expire and be rejected.

Lastly, you can enter your payment by credit card in the space provided.

Click Continue, this will complete your application and submit it to LoTW.

One more very important item:

After clicking Continue the next screen will confirm that your application is submitted. On this screen is a link that YOU MUST click. It is calledPrint Field Checked List. This is ONLY needed for Hybrid applications and this MUST accompany your paper cards to the HF Awards Manager, or, if you send your paper application to ARRL. For hybrid applications the old paper application and record sheet are totally unuseable. The cards and printout direct from LoTW are the only requirement.
