You must qualify for 5B WAS with 50 states on each of the 5 primary bands of 80, 40, 20, 15 and 10 meters.

• Sign on to the LoTW site then select "Awards"
• Click "Select WAS Award Account"
• Click "Application" to start the application process
• Select each of the 5 primary bands as endorsements only
• Click "Continue"
• Part 2: Check the "Endorsement" button
• (If applicable) Complete the paper portion by noting the callsigns in the box(s) next to each of the State names corresponding to the proper band(s)& note if you are sending cards to ARRL HQ or an HF Awards Manager
• Click "Continue"
• Part 3: Complete the personal information LoTW needs for your application
• Click "Continue"
• Part 4: Place note in the comment box: "This application is only for 5 Band WAS," if you would like to purchase the (optional) wall plaque and lapel pin, and any additional comments.
• Payment
  
  Note: Free awards apply only to level 4 or higher in the ARRL Diamond Club or Maxim Society.
  
  Payments by mail/fax must be received at the WAS desk within 30 days. If payment is not received within 30 days your application may expire and be rejected.

• Click Continue, this will complete your application and submit it to LoTW.
• Print the Field Checked List and send with your paper cards to the ARRL or the HF Awards Manager.
